Payments and Billing Information
- The initial office visut is $150.00
- Follow up sessions are $120.00
- We accept most insurance, EAP, and SAP coverage
- Please contact your carrier in regard to your deductibles and copayments
- Bring your insurance card and/or EAP/SAP authorization number
- Employee Assistance Program (EAP/SAP) benefits must have prior authorization
- We do not accept credit/debit cards, bill pay, MSA, etc.
- Payments must be made with cash, check, or money order at the time of your appointment
APPOINMENT PROTOCOL
- Appointments may be self referrals, insurance, EAP, or medical
- Call to arrange an appointment and we will confirm via email
- Documents and information forms must be completed and returned
- We do not have walk-in appointments
- The first appointment is for the parent(s) only
- Appointments are approximately 50 minutes
Office Closures
There are some circumstances when our office is closed. When possible, we notify our active clients. This may include:
- Federal/state holidays
- Hurricanes
- Civil unrest
- Damage or flooding to the office or parking area
